Public Event & Photo Liability Waiver Notice

This waiver serves as a public disclaimer for all participants, guests, and volunteers attending or engaging in any public or private event, meeting, or activity organized by the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated.

By participating in chapter activities, individuals acknowledge and accept the terms outlined in the Public Event & Photo Liability Waiver. This document is not a chapter policy, but a standard web platform disclaimer designed to ensure transparency, safety, and mutual understanding regarding participation, photography, and media usage.

The waiver outlines the scope of consent, photo and media release, emergency authorization, and general liability protection applicable to all chapter-sponsored activities.

LIABILITY WAIVER AND RELEASE OF CLAIMS
This waiver applies to all public and private events, meetings, and volunteer activities organized, hosted, or promoted by the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated.

I acknowledge that I derive personal satisfaction and benefit from participating in events held, sponsored, or promoted by the Chapter, and I willingly engage in such events.

RELEASE AND WAIVER
I hereby release, waive, and forever discharge any and all liability, claims, and demands of any kind against the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated, its affiliated partners, sponsors, and property or site owners of any activity, including without limitation their directors, officers, employees, volunteers, and agents (collectively, the “Released Parties”). This release applies to any claims, whether in law or equity, to the fullest extent permissible by law, including but not limited to claims for death, bodily injury, illness, economic loss, property damage, or other losses that I, my heirs, assignees, next of kin, or legal representatives may have or may later accrue as a result of my participation in any chapter-related activity.

PHOTOGRAPH AND PUBLICITY RELEASE
I grant permission to the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated to use my name, likeness, image, voice, and/or appearance, and that of my minor child or dependent (if applicable), in any photographs, video recordings, audiotapes, digital images, or similar media (“Material”) created on behalf of the Chapter.

I irrevocably assign to the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated full ownership and copyright of such Material without compensation, including the right to edit, reproduce, or otherwise use the Material for educational, promotional, and marketing purposes. This includes, but is not limited to, publication on webpages, social media, print materials, digital communications, and news media. This release is perpetual and applies to current and future uses of the Material without limitation.

I release the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated, its representatives, licensees, and assigns from any and all claims connected to such use, including but not limited to copyright, privacy, or publicity claims under federal or state law.

ASSUMPTION OF RISK AND COVID-19 EXPOSURE
I acknowledge that participation in any event held, sponsored, or promoted by the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated may expose me to infectious diseases, including but not limited to COVID-19. I understand that, while preventative measures are in place, the Chapter cannot guarantee that participants, volunteers, partners, or others in attendance will not contract COVID-19 or another communicable illness.

By attending or participating, I knowingly and freely assume all associated risks, including illness, injury, or death, regardless of negligence by any Released Party. I agree to follow all published or communicated safety rules and guidelines for each venue or event.

By attending or participating in any event or activity hosted by the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated, I acknowledge and accept full responsibility for any potential injury, harm, or loss. I further release the Chapter, its members, event site owners, sponsors, volunteers, and hired staff from any liability to the fullest extent permitted by law.

EMERGENCY MEDICAL AUTHORIZATION
In the event of a medical emergency, I authorize the Eta Omega Zeta Chapter of Zeta Phi Beta Sorority, Incorporated or its designated representatives to obtain reasonable first aid or emergency medical treatment for me (or my minor dependent) if necessary. I understand that I am responsible for all resulting medical expenses.

BY ATTENDING AND/OR PARTICIPATING IN ANY EVENT OR ACTIVITY HOSTED BY THE ETA OMEGA ZETA CHAPTER OF ZETA PHI BETA SORORITY, INCORPORATED, YOU DO SO AT YOUR OWN RISK AND ARE DEEMED TO HAVE PROVIDED A FULL RELEASE OF LIABILITY TO THE CHAPTER AND ALL ASSOCIATED PARTIES.
